You are required to complete the Intake Forms within ONE WEEK! This is necessary to allow appropriate prioritization of the referral and securing an appointment time for you!

The Intake Forms may be completed:

Electronically by using the fillable PDF file (requires latest Adobe PDF Reader); or
By pen or pencil (requires printing a hardcopy).
