How do I get an appointment with Dr. Bordbar?
You will need a referral a physician. Your physician may use his/her own referral request form. Alternatively, your physician may use Dr. Bordbar Referral Form. Referrals can be faxed to 416.548.9702.
What documents are needed for the initial appointment?
- Valid provincial Health Card (OHIP Card)
- List of medications (current and previous)
- Copies of relevant documents such as previous assessments by Health Care Professionals
- Previous assessments by Health Care Professionals
- Psychological and Academic Assessments
- School Report Cards
- Individual Educational Plan (IEP), etc
Who should come to the appointments?
All Children and Adolescent must be accompanied by at least one parent or legal guardian
What is the cancellation policies?
At least two business days notice is required to avoid the Administrative Fee. Please see Unicare Policies and the Fee Schedule for details.
What is the administrative fee payable for missed appointments or late cancellations?
