You will be updated with the status of your request via email as soon as your request is processed.
Please allow up to one week to receive a response.
Initial Appointment for New Patients
A referral by your doctor is required before the first appointment can be booked!
Your doctor may send his/her own referral request or alternatively use our Referral Form. Once the referral request is received, you will be contacted with further instructions on how to book your first appointment.
Follow-up Appointment for Existing Patients
You may book a follow-up appointment if you are an existing patient.
