How do I get an appointment with one of the specialists at Unicare Clinic?

You will need a referral by a physician. Your physician may use his/her own referral request form. Referrals can be faxed to 416.548.9702.

What documents are needed for the initial appointment?

  • Valid provincial Health Card (OHIP Card)
  • List of medications (current and previous)
  • Copies of relevant documents such as previous assessments by Health Care Professionals

How much notice is required to reschedule or cancel my appointment?

At least two business days notice is required to avoid the Administrative Fee.

What will happen if I miss my appointment (no-show) or don’t give enough notice to cancel or reschedule my appointment?

Missed appointments or late cancellations will incur the Administrative Fee.

What will happen if I arrive late for my appointment?

Late arrival of 20 minutes after the scheduled appointment time is considered a no-show and will be subject to the Administrative Fee..

Why is there a cancellation policy?

An appointment is booked specifically for you, which eliminates the opportunity for other patients to receive the care that they equally require. As a courtesy to your fellow Ontarians and your physician, at least two business days notice for any cancellations or changes will be necessary. The Administrative Fee. will be charged to cover the cost of staffing and preparing the office for your appointment.

Is there a fee for services not covered under OHIP (Uninsured Services)?

Yes. You are required to pay for services not covered under OHIP (Uninsured Services), such as sick notes, prescription renewals without an office visit, preparing specific letters, completing forms, copies of medical records, etc.Please see the Fee Schedule for details.
